Please follow this checklist for all employee paperwork required by Central Office for new hires: Employment Application Checklist

Non-Certified Application (i.e., Support Staff)

Certified Application (i.e., Teachers/Administrators)

Forms for all new employees, please fill out attach and submit with application:

  1. I-9 Form

  2. I-9 Instructions

  3. Indiana WH-4 Form

  4. Federal W-4 Form

  5. Direct Deposit Authorization

  6. IN New Hire Form

  7. Criminal History Background Check - or provide a copy of one that has been completed within the last five years.

To Become a Substitute:

Obtain a Substitute License through the state at https:/license.doe.in.gov/ (or a current Teacher’s License).

  1. Complete Shelby Eastern's Substitute Application and all general employment forms above.

  2. Complete a Criminal History Background Check - or provide a copy of one that has been completed within the last five years.

  3. Turn in all paperwork to the Central Office at 2451 N 600 E, Shelbyville, IN 46176.

  4. The Substitute Caller will call you to obtain details about your subbing preferences and contact method.
